How to make an inquiry
If you are interested in scheduling a consultation, I can be reached via phone at (315) 956-4236, or you can e-mail me at [email protected].
In your e-mail, please include:
1) your name,
2) your phone number,
3) your preferred appointment times, and whether you have daytime availability,
4) the reason(s) you are interested in a consultation and/or seeking treatment,
5) For clients for whom I am an in-network provider, the name of your insurance carrier.
For clients for whom I am an out-of-network provider, the name of your insurance carrier and your out-of- network benefits.
In-network clients
I am a participating provider for:
- Excellus Blue Cross Blue Shield (New York only)
- Optum (New York and Connecticut)
Out-of-network clients
Out-of-network benefits are typically in the Summary of Benefits on the insurance company’s website.
If you contact them, you will want to ask:
1) Is there a deductible, a co-pay or co-insurance to see a psychologist who is not in-network?
What is (are) the amount(s)?
2) If there is a deductible, once it is met, what percent of services will they cover?
3) Where does one find the medical claim form and/or submit claims online?
I will provide you with detailed receipts so that you can submit them to your insurance company for reimbursement.
Self-pay clients
The initial consultation is $250.
The 55-minute therapy session is $200.
The 45-minute therapy session is $175.
The 30-minute therapy session is $100.
